The team "England A" was formed in 1999 to contest the European Cup, which England had won four times already. The second side did just as well by winning the fifth edition. In 2001, the tournament was contested by the England - under 19 side. In 2005, it was the turn of the "England Development Squead", which I equate with the England A side.
Also included in these ratings are the results from the IWWC (now considered ODI's), and the European Championships, in which Wales made their first appearance in 2005.

Only nine nations play four-day matches against each other. The West Indies played a few series in the 80's, and have now played Pakistan.
Ireland and Sri Lanka have also only played Pakistan, but of these four only
the West Indies very long ago met any of the top five, so the ratings are
separate for two groups.

I realize quite well that Ireland's rating seems ludicrously high. However, who is to say they would not have scored 500 runs in each innings (they declared at 193 for 3) against Pakistan (who themselves were bowled out for 53 and 86). But of course these ratings do not work on just two matches.
